A form was sent during the APTA Utah spring conference for virtual attendees to fill out confirming attendance.All quizzes have been assigned and can be found here (you must login): LINK

DIRECTIONS & PARKING MAP
THOSE ATTENDING IN PERSON - You will need to check-in both days for your in person attendance. Those who are not there in person and check-in will not receive Category I CEU.
VIRTUAL ATTENDANCE - In order to receive category II CEU you will have to complete a post-conference quiz for each presentation you attended. You will receive a form to indicate which presentations you attended, then a link to those will be emailed to you. If you do not wish to receive Category II CEU, you will not need to complete the post conference quiz. Please make sure katie@aptautah.org is added as a safe sender and my emails are not going to junk.

- The attendee portal link is on your confirmation email with your confirmation code. This portal will have all event details and links to the survey & questionnaire.

- Once you fill out the post conference survey, your certificate will be in your APTA Utah account. APTA Utah members and non-members both have APTA Utah accounts. We are not able to print out or email certificates.We appreciate your patience as this is a new certificate process with the new CEU rules.
- You will need to sign in and view your account.

- Click Certificates
- Access to your certificate will be at this link once your attendance is confirmed: LINK
- You will need to take the post-conference survey & the post conference questionnaire if you attended virtually and want category II credits.
- To verify attendance, please contribute to the online discussions. Using the term checking in or here doesn't qualify as a contribution.
- TECH ISSUES
- Stream is frozen - Please refresh your browser.
- Echoing or multiple sounds - Please make sure you only have one browser stream open.
- Stream not loading - Please refresh your browser, close and re-open it, or use a different device/browser.
- Sometimes the stream will freeze briefly, just wait a second or refresh your browser.
- There is not a password for any of the streams.
- The chat is at the bottom of the stream. Type in your name in the line above the happy face and your message on the happy face line. :)
